Yes No Comment <br />Contingency Plan and Emergency Procedures (cont.) <br />Chapter 15, Article 4 <br />If an emergency occurred at this facility, did <br />the EC immediately: [66265.56-] <br />(a) Activate alarm systems and notify <br />appropriate response agencies? — <br />(b) Identify the character, source, amount and <br />areal extent of released materials? _ <br />(c) Asses the possible direct and indirect <br />hazards from the release, including gases and <br />run-off of fire fighting materials? — <br />(d-) When assessment determines that the release <br />could threaten human health or the envirorment <br />outside the facility: <br />(1) Report his findings to appropriate <br />authorities if it may be advisable to <br />evacuate the local area, and remain on call <br />to help authorities decide? — <br />(2-) Immediately notify the state office of <br />Emergency Services with a report including <br />required information? — <br />(e) During emergency, does EC take all <br />reasonable measures to minimize release? — <br />(f) If the facility had to stop operations to <br />respond, does the EC monitor all appropriate <br />equipment? — <br />(g) After the emergency, does the EC immediately <br />provide for the TSD of recovered or contaminated <br />material resulting from the release? — <br />(h-) Does the EC ensure that in the affected <br />areas of the facility: <br />(1) Wastes incompatible with released <br />material are not handled until after clean-up <br />is complete? — — <br />(2) All emergency equipment listed in <br />contingency plan is clean and fit for use <br />before operations r -es mme7 — <br />(i) Does the generator notify the Department, <br />state and local authorities that 66265.56(h) is <br />complied with before resuming operations in <br />affected areas? <br />(j) If the contingency plan was implemented, did — the generator record details of the incident in <br />the operating record arid, within 15 days, submit <br />a written report to the Department? — <br />Revised 7/15/91 Page 24 <br />
