The Spill Prevention, Control, and Countermeasure <br />(SPCC) Plan was amended on 7-1-2013 to include the following change: a 240 -gallon temporary transfer diesel tank <br />was removed from the facility (taken out of service in 2013 and picked up by a local farmer to be used as a diesel <br />tank). This amendment is a technical change, requiring a PE certification within six months of amendment. A <br />technical amendment is a change to the facility, tanks, procedures, materials, construction, design, or maintenance <br />that materially increases or decreases the facility's potential for oil discharge. Immediately obtain a complete PE <br />certification for the facility's SPCC plan. Submit a copy of the completed, certified SPCC plan to the EHD for review. <br />This is a Class II violation. <br />604 <br />CFR 112.7(a)(3) No facility diagram or didn't show location and contents of containers, transfer stations, and pipes. <br />No underground storage and no connection piping for all facility transfer lines were included on the facility diagram. <br />The Spill Prevention, Control, and Countermeasure (SPCC) Plan shall include a facility diagram which must mark the <br />location and contents of each fixed storage container and the storage area where mobile or portable containers are <br />located. It must identify the location of and mark as "exempt" underground tanks. It must also include all transfer <br />stations and connecting pipes, including intra -facility gathering lines. Immediately update the facility diagram to <br />include all of the required information. Submit a legible copy of the updated facility diagram to the EHD for review. <br />This is a Class II violation. <br />613 <br />CFR 112.7(e) Failed to conduct inspections or maintain records for 3 years. <br />Copies of inspection and testing records (as required by the SPCC plan) were not found on site. Inspections and <br />tests must be conducted in accordance with the written procedures developed in the Spill Prevention, Control, and <br />Countermeasure (SPCC) Plan. Records of these inspections and tests must be signed by the appropriate supervisor <br />or inspector and kept on site with the SPCC Plan for a period of three years. Immediately locate a copy of all <br />inspection and testing records for the last three years, maintain them on site, and submit copies to the EHD. <br />This is a Class II violation. <br />614 <br />CFR 112.7(f)(1) Failed to train personnel on discharge prevention. <br />Oil handling personnel training records are not maintained as required by the onsite SPCC plan; also the SPCC plan <br />indicated that fuel delivery vehicle operators will be trained with respect to the location and use of the onsite spill kits <br />and spill notification requirements. At a minimum, oil handling personnel shall be trained in the operation and <br />maintenance of equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, <br />rules, and regulations; general facility operations; and the contents of the Spill Prevention, Control, and <br />Countermeasure Plan. Immediately provide this training to all oil handling personnel and submit a copy of the training <br />log to the EHD. <br />This is a Class II violation. <br />Page 4 of 5 <br />
