SUMMARY OF VIOLATIONS CLASS I, CLASS II, or MINOR -Notice to Comply) ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION Class 11 violations: (121, 122, 123, 124, 404, 407, 605) 121. Manifest number 004824261JJK (11/6/09) was found on site at the facility but was not on record with the Department of Toxic Substances Control (DTSC). Within 30 days of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. Immediately submit a copy of this manifest to DTSC and ensure that copies of all future manifests are submitted to DTSC within the required time frame. REPEAT VIOLATION 122, 123, & 124. Manifest number was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
