Four drums of hazardous waste and additional <br />hazardous waste that would not fit in the full drums were observed on site as part of the reverse logistics hazardous <br />waste collection from wastes found in boxes shipped to the facility from individual stores. No owner or operator shall <br />of a facility shall accept, treat, store, or dispose of a hazardous waste at the facility unless the owner or operator <br />holds a hazardous waste facilities permit from the Department of Toxic Substances Control (DTSC). Immediately <br />stop receiving hazardous wastes generated from off-site facilities. <br />This is a Class I violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br />complete report which replaces the initial checklist. Violations #106, 107, 201, and 3050 have been added. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />*According to Mr. Tony Evans, the facility began accumulating hazardous waste from reverse logistics activities in <br />August 2015 and have not yet had any of the waste hauled away. <br />Waste streams generated at this facility: <br />- Used oil - Aerosols <br />- Used antifreeze - Fuel contaminated water <br />- Absorbent - Reverse logistics waste <br />Provided: Return to Compliance Certification form, Modified Contingency Plan template, Schedule of free CUPA <br />classes <br />AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING THIS <br />FACILITY BACK INTO COMPLIANCE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Received by: Date: <br />Inspector: <br />Inspector Phone: <br />` Printed Name and Title: <br />/0'v le <br />STACY RIVERA, Senior REHS <br />(209) 468-3440 <br />Page 9of9 <br />
