Exposure incident investigations: <br />♦ Employees involved in the incident. <br />♦ Date and time of the incident. <br />♦ The exact location of the incident. <br />♦ The type of potentially infectious material involved. <br />♦ The circumstances surrounding the incident that led to <br />exposure (i.e. who was doing what). <br />♦ Recommendations to prevent similar exposure incidents. <br />♦ The Risk Management Department, Insurance Company or <br />Clinic will maintain all medical records relating to the exposure <br />incident, including: <br />1. The Health Care Provider's opinion if Hepatitis B <br />vaccination is recommended. <br />2. The Health Care Provider's statement that: <br />a. the employee has been informed of evaluation <br />results; <br />b. the employee has been advised of future recommended <br />treatment; and <br />c. the signed declination form if the exposed employee <br />declines an HBV shot. <br />Training records will be made available for examination and <br />copying by employees, their representatives, and OSHA personnel. <br />Never let the records leave the Store or Distribution Center. <br />Toys "R" Us, Inc. <br />HAZARD COMMUNICATION PROGRAM BLOODBORNE PATHOGENS <br />9/1/95 <br />
