OZaeegs Family of Fine Stores August 11, 2011 Garrett Backus Environmental Health Department 600 East Main Street Stockton, CA 95202 RE: Response to Underground Storage Tank (UST) inspection report of July 18, 2011 at Raley's Aisle 1 #356 4219 Morada Lane, CA 95212 Dear Mr. Backus, This letter is in response to the Department's July 18, 2011 UST inspection report. Each numbered item below corresponds to the numbered items in the report summary of violations. Item #4 Certification of Financial Responsibility is on file at this site and a copy is attached. Item #7 Maintenance records for the following two alarms were not found on site: Q4 -diesel PLLD 10-22-10 positive shut down alarm and S4-91 product on 10-28-10 communication alarm. Attached is a copy of the designated operator monthly visual inspection for October the Q-4 diesel PLLD alarm is noted on page 2. Also attached is a copy of the maintenance record invoice #1122869 for the repair of this alarm. You will see that the date at the top of invoice #1122869 is 10-21-10. However, the technician did sign this invoice on 10-22-10 when the work was performed. This document was on site at the time of inspection but may have been missed due to the date discrepancy. We cannot find any record of the system having a 54-91 product communication alarm on 10-28- 10. However, we do see a record on the veeder root printout and also on the November UST monthly inspection of an L20 fuel alarm, which was corrected by Service Station Systems on 10- 28-11 invoice #1121746. Inspection report and copy of invoice are attached. Item #18 was noted in the report as corrected on site. Item 23c Designated operator failed to document S4-91 product communication alarm dated 10-28- 10. Since being notified of this we have been working with our Designated Operator (Service Station Systems) and reviewing our October and November UST monthly inspection reports along with the veeder root print outs of all alarms that occurred and we are not able to find a 54-91 product communication alarm on 10-28-10 ever occurring. We have found a L20 fuel alarm 10- 28-10 that was documented on the November UST monthly inspection report and also corrected by SSS on 10-28-10. Attached is a copy of the November inspection report and invoice #1121746 for this L20 fuel alarm on 10-28-10.
