Replacement piping for the satellite #1 piping run was installed on June 2012 and six month testing <br />was due by December 2012. Testing has not been completed and is three months past due. <br />Secondary containment testing is required six months after installation and at least every 36 months <br />after. Immediately schedule this test and provide 48 hours notification to the EHD. <br />108. Secondary containment testing was performed on the satellite #1 piping run on June 7, 2012 and a <br />test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 <br />days of the test. Immediately submit a copy of the test report to the EHD. <br />115. Piping replacement was done on the satellite #1 piping run on June 7, 2012 and tightness testing <br />results have not been submitted prior to operating the tank. Immediately provide copies of the test <br />results to the EHD. <br />301 & 307. The January 10, 2012 secondary containment test results indicate a failure at UDC 3/4. <br />EHD has not received notification of any additional secondary testing for this UDC. A permit has been <br />issued by EHD for repairs relating to other failures during the January 2012 secondaryc n jnt(�} <br />testing, however, this permit addresses only the possible removal and reinstallation of W.3/4, and' <br />does not address any permit -required repairs to UDC 3/4. The underground storage tank system shall <br />be operated to prevent unauthorized releases. All secondary containment for the UST system must be <br />tight.
