The business was Oilstop, a drive thru oil <br />change facility that also services transmissions and cooling systems. <br />On October 6, 2011, Mr. Cacapit spoke with Mr. Thomas Lyon of Oilstop and asked him <br />to provide his contact and facility information. Mr. Cacapit sent Mr. Lyon a fact sheet, <br />"Hazardous Waste Generator Requirements," from the Department of Toxic Substances <br />Control (DTSC), by email (Attachment 1). <br />On October 19, 2011, Mr. Cacapit performed a routine hazardous waste inspection and <br />found that a contingency plan and personnel training records were not on site <br />(Attachment 2). The facility generated approximately 3.37 tons of hazardous waste <br />since they started operations earlier in the month, which is in excess of 1,000 kg, making <br />them a large quantity generator. <br />On October 20, 2011, Mr. Lyon submitted the facility's EPA ID number (Attachment 3). <br />On November 20, 2011, Mr. Lyon sent an email to Mr. Cacapit stating the actions he <br />performed to correct the violations from the October 19, 2011, hazardous waste <br />inspection (Attachment 4). The email also stated that the supporting documents for the <br />return to compliance were sent in the mail. <br />On November 22, 2011, a corrective actions statement, return to compliance <br />certification, and supporting documents were submitted in response to the October 19, <br />2011, hazardous waste inspection (Attachment 5). <br />
