The revised MRP and figures will need to show the changes. Thanks.
Jim Barton
From : Daniel Villanueva [mailto:dvillanueva(aadvgeoenv.com]
Sent: Thursday, February 05, 2015 10:44 AM
To: Angelica Sandoval Marin [EH]; 'Barton, Jim@Waterboards' (jim. barton(&waterboards.ca.gov)
Cc: John Yoakum [EH]; Brian Millman; William Little
Subject: Corrected Work Plan for Apache Plastics
Jim and Angelica,
As previously mentioned in past correspondence, errors were present in both the approval letter and
the AGE prepared, Waste Discharge Permit Application — Addendum & Well Installation Work Plan,
dated 14 November 2014.
In the work plan the body of the text incorrectly identified the proposed number of wells. The work plan
called out the installation of five additional wells. However, the site plan, previously approved in email
correspondence with Mr. Mike Smith of the Regional Board, correctly identified the number of wells as
6 (two deep injection wells, one deep monitoring and three shallow monitoring wells [original work
plan text only identified 2] ). These errors were also present in the Regional Board approval letter. I had
spoken with Jim following receipt of the letter and we identified that both the work plan and the
approval letter text were in error.
As such, I have prepared a brief addendum correction letter (attached ), which correctly identifies the
number wells, locations and procedures for installation . Permits for the installation have been
submitted and the work, as previously mentioned, is scheduled to begin on Monday 09 February 2015.
Please let me know if there are any questions regarding the attached work plan correction letter and if I
may proceed with the proposed schedule .
Regards,
