301 West Kettleman Lane, Lodi - 2 - , 22 September 2000 <br />Wased on this information, additional site work will need to be completed, and a revised PAR and FRP <br />submitted. Therefore, you must complete the following: <br />1. By-" <br />ctpber1Q0Q,--submit a report to this office documenting actions completed to stabilize the <br />groiuid ter Vui ie at .the site. <br />2• By qaf�41 10, submit a plan of work to delineate the lateral and vertical extent of groundwater <br />contamination both on-site and off-site, and replace MW -8B. This work must include the use of <br />continuous coring methods at all soil borings to evaluate the subsurface geology prior to installing <br />monitoring wells. <br />3. By 4 December 2000, submit a complete Problem Assessment Report/Final Remediation Plan as <br />described in Appendix A of the Tri -Regional Recommendations. The following information must be <br />included: <br />a. Figures delineating horizontal and vertical extent of groundwater contamination. <br />b. Boring logs and well completion diagrams for all borings and wells completed for this <br />investigation, including replacement well MW -813. <br />c. Detailed north -south and east -west cross-sections, and a cross-section from monitoring well <br />MW -4 to City of Lodi Municipal Well 12 showing extent of soil and groundwater contamination. <br />d. A groundwater flow diagram to document historic flow directions and magnitude. <br />. e. Preliminary pumping test data, as necessary for remedial alternatives. <br />f. A table to list all monitoring well details including: well number, date installed, casing diameter, <br />casing material, slot size, surveyed elevation, top and bottom of screen, top and bottom of filter <br />pack, depth to water, groundwater elevation, and aquifer zone. <br />g. A complete list of groundwater analytical data. <br />During completion of these documents, active soil vapor extraction and groundwater extraction must be <br />continued. The groundwater extraction must stabilize the groundwater plume to prevent movement of <br />MtBE toward Well 12. If you satisfactorily complete the above items you should be in compliance with <br />C&A Order 5-00-704. Should you fail to meet the dates in this Notice of Violation, you will be in <br />continued violation of the C&A Order, and any fines will be based on the original dates listed in C&A <br />Order No. 5-00-704. <br />If you have any questions or comments, please call Marty Hartzell at (916) 255-3115. <br />WILLIAM .
