California 95202-3029 <br /> Telephone: (209) 468-3420 Fax: (209) 468-3433 Web:vwvw.sigov.org/ehd/unitiii.html <br /> CONTINUATION FORM Page: 4 of 5 <br /> OFFICIAL INSPECTION REPORT Date: 03/08/11 <br /> Facility Address: Safeway Distribution Center 16900 West Schulte Road, Tracy Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> Routine Aboveground Petroleum Storage Tank Inspection <br /> 18, 19, 27. Facility has a 500 gallon diesel tank emergency generator located outdoors by the grocery <br /> building that was not on the diagram and not found on the inventory. There is also another diesel tank <br /> emergency generator with an unknown tank shell capacity located outdoors by the produce building. <br /> Discharge & drainage controls and potential equipment failure are not addressed for these unlisted <br /> containers. The SPCC Plan must include the correct type of oil and storage capacity of each fixed <br /> storage container, and either the type of oil and storage capacity of each portable container or an <br /> estimate of the potential number of portable containers, types of oil, and anticipated storage capacities. <br /> The Plan must include a prediction of the direction, rate of flow, and total quantity of oil which could be <br /> discharged as a result of each type of major equipment failure. Immediately update the inventory and <br /> discharge information in the SPCC Plan to accurately represent the petroleum storage at the facility. The <br /> plan shall include a facility diagram which must mark the location and contents of each fixed storage <br /> container and the storage area where mobile or portable containers are located. It must identify the <br /> location of and mark as "exempt" underground tanks. Correct by updating the facility diagram, inventory <br /> and the prediction discharge to include all of the required information for the missing tank(s). <br /> 24. Plan is missing phone number for the local CUPA (Certified Unified Program Agency): SJC EHD on <br /> Table 4.1 Contact List. The SPCC Plan must include a contact list and phone numbers for the facility <br /> response coordinator, National Response Center, cleanup contractors with whom you have an <br /> agreement for response, and all appropriate federal, state, and local agencies who must be contacted in <br /> case of a discharge. Immediately update the contact list to include the most current version of the <br /> required information. <br /> 31. In Section 3.3, the Professional Engineer (PE) stated that the facility will conduct several types of <br /> inspections: daily in the oil room, weekly visual at the fuel island, a monthly general SPCC inspection of <br /> tanks and containers areas and conduct annually check liquid level gauges/ sensing devices and <br /> alarms. Daily and weekly inspection reports were available for review but monthly and annual inspection <br /> reports were not available since they were not conducted. Inspections and testing shall be conducted on <br /> all aboveground liquid petroleum containers larger than 55 gallons, including all the tanks and all 55 <br /> gallon drums . Records of these inspections and tests shall be signed by the appropriate supervisor or <br /> inspector and kept on site with the SPCC Plan for a period of three years. Immediately begin necessary <br /> testing and inspections for all APSA regulated containers and maintain on site with the SPCC Plan or <br /> have the PE certified on the types of inspections and procedures. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD spectar Received icy: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
