Plans stated Director of Operations Jack Mixey is the designated personnel in charge of oil spill <br /> prevention and is the trainer for oil-handling operations for employees. Annual training is not being <br /> conducted for the aboveground petroleum storage program and containers. No aboveground petroleum <br /> storagetraining records can be found. At a minimum, oil-handling personnel must be trained in the <br /> operation and maintenance of equipment to prevent discharges, discharge procedure protocols, <br /> applicable pollution control laws, rules & regulations, general facility operations, and the contents of the <br /> facility's SPCC Plan. Annual briefings must be conducted to assure adequate understanding of the <br /> SPCC Plan and highlight and describe any known discharges or failures, malfunctioning components, <br /> and any recently developed precautionary measures. Immediately conduct necessary training for all oil <br /> handling personnel by the designated trainer or have the PE revised whom would be the designated <br /> trainer for oil spill prevention. Submit copies of oil-handling SPCC training records to the EHD by <br /> 04/17/11. <br /> 52. Failed to promptly correct visual discharges. One of the hoses near tank 3 has a slow drip of diesel <br /> that accumulates into an open container of oily-water. This leakage was pointed out to ARC supervisor <br /> Curt. The accumulation can hide potential small discharges that are unseen. Correct by promptly <br /> correcting visible discharges that results from loss of oil from container, including but not limited to <br /> seams, gaskets, piping, pumps, valves, rivets and bolts. Collected waste should be handled in <br /> accordance to California Code of Regulations, Title 22, Hazardous Waste regulations. Correct by <br /> repairing the equipment to stop the dripping of diesel from the hose. <br /> Submit to SJC EHD within 30 days (04/17/2011) <br /> 1. one copy of the complete "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. supporting or requested documents <br /> Note: <br /> 20 year formal external inspection of outdoor diesel fuel tank by certified contractor following STI <br /> standard due 2012.
