G, Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> -- Annual site-specific training is not being conducted for the aboveground petroleum storage program <br /> and containers. At a minimum, oil-handling personnel must be trained in the operation and maintenance <br /> of equipment to prevent discharges, discharge procedure protocols, applicable pollution control laws, <br /> rules & regulations, general facility operations, and the contents of the facility's SPCC Plan. Annual <br /> briefings must be conducted to assure adequate understanding of the SPCC Plan and highlight and <br /> describe any known discharges or failures, malfunctioning components, and any recently developed <br /> precautionary measures. Immediately conduct necessary training for all oil handling personnel. Submit <br /> copies of training records to the EHD. <br /> -- This facility does not have a designated person for discharge prevention. The SPCC Plan shall <br /> designate a person at the facility who is accountable for discharge prevention and who reports to facility <br /> management. Immediately designate a person to be accountable for discharge prevention and update <br /> the SPCC Plan to include this information. <br /> -- The SPCC Plan did not include a complete discussion of site security. The Plan must include <br /> descriptions of how you secure and control access to the oil handling, processing & storage areas, <br /> secure master flow & drain valves, prevent unauthorized access to starter controls on oil pumps, secure <br /> out-of-service and loading/unloading connections of oil pipelines, and address the appropriateness of <br /> security lighting to both prevent acts of vandalism and assist in the discovery of oil discharges. <br /> Immediately update the Plan to include all of the required security information. <br /> -- The description of secondary containment capacity does not address freeboard to contain <br /> precipitation. All bulk storage tanks must be provided with a secondary means of containment for the <br /> entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide <br /> sufficient secondary containment for all tanks at this facility. <br /> -- Integrity testing is not addressed in the SPCC Plan. The facility shall test or inspect each <br /> aboveground container for integrity on a regular schedule and whenever you make material repairs. <br /> Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic testing, <br /> radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive <br /> testing. Comparison records and other records of inspections and tests must be maintained on site with <br /> the SPCC Plan. Immediately conduct all necessary integrity testing and maintain records on site as <br /> required. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title:EHD 23-02-003'V07'11 2/11 CONTINUATION FORM <br />
