El Dorado St.—4/25/2011 <br /> Dear Ms.Rivera: <br /> Further to our letters dated May 26,2011,and June 15,2011,responding to violations re the HW <br /> Program Inspection on April 25,2011,the following additional changes/corrections have been <br /> made: <br /> Waste Minimization—a completed Onsite Hazardous Waste Treatment Notification is enclosed. <br /> Tank and Containment Assessment—tank integrity testing includes a visual inspection and <br /> hydrostatic test(tank filled with liquid).This is consistent with both the Model Tank and <br /> Secondary Containment Assessment Document,and with 40 CFR 112.8 requirements for <br /> preparation of a Spill Prevention Control and Countermeasures Plan(SPCC)given in 40 CFR <br /> 112.8(c)(6). <br /> Personnel Training Records—personnel hiring is done through the Machinists Union for the job <br /> classifications listed on the Safety Training Sign-in sheet.All personnel must speak,read and <br /> write English,and all receive on-the-job training from company supervisors and/or trainers <br /> during a 90 day probationary period prior to hiring by the company.Initial training and refresher <br /> training(quarterly)is provided as outlined in the Injury and Illness Prevention Program(IIPP), <br /> summary attached. The company is currently developing a training data base to summarize the <br /> training records,to ensure that each employee receives refresher hazardous materials training <br /> within the time frame required by the regulations. <br /> Aboveground Petroleum Storage—the Spill Prevention Control and Countermeasures Plan <br /> (SPCC)has been revised to correct the deficiencies cited.A copy of the corrected plan is <br /> enclosed. <br /> Thank you for working with us to meet all of the regulatory requirements.We believe that we <br /> have responded to all of the issues cited. <br /> Sincerely, <br /> Jack Bishop <br />
