JUN 2 U Z014 5.0 OWNER RESPONSIBILITIES: ENVIRONMENTAL HEALTH 5.1 It is the customer's responsibility to determine if this scope of waWAR plan or necessary requirements 5.2 For insulated tanks the owner must ensure that the insulation does not include asbestos prior to scheduled inspection. 5.3 Provide technician with reasonable access to the AST's to be inspected. Inspections will be completed during normal business hours. 5.4 Free and clear access to the tanks,piping and containment. 5.5 The tank surfaces and welds to be inspected by visual or ultrasonic examination must be free from scale,rust,dirt,oil,grease or and other material they may interfere with these 5.6 examinations. 5.5 Reasonable access to the top of the tanks for the inspection of the roofs. 5.6 Provide previous monthly and annual inspection records. 5.7 Any repair or alteration records for the AST system. 5.8 Provide any information by the tank manufacturer(s)to include original tank design specifications and manufacturers standards The Following Items Are Not Included in Proposal 1. Additional work requested outside the scope of this proposal will be billed at time and material. 2. Any required safety training. Safety training will be billed at$75/per man hour. 3. Any state or local permits. 4. Dewatering,storage or disposal of water. 5. Repair or replacement of conduits. 6. Environmental evaluation,assessments or testing 7. Excavating,loading,handling,disposal or backfill needed due to contaminated soil or water. 8. Asphalt patching or replacement. 9. Landscaping restoration,seeding or sod. 10. Freight unless specifically noted. 11. Applicable sales tax unless specifically noted. This price includes all living,travel, and incidental expenses incurred by Tanknology's engineers/technicians while executing the proposed work. The price also includes the cost of drawings, reports and manuals, and includes use of specialized test instruments and laboratory tests as may be needed to accomplish the proposed work. No additional charges will be made by TANKNOLOGY except as provided in Section 5 or Section 11 or as otherwise specifically set forth herein. However, the above price does not include any duties or applicable sales or use taxes, nor does the price include any fees,permits, licenses or the like,which shall be borne by the Purchaser. 6.0 TERMS: 50%down payment prior to ordering equipment or scheduling work. The balance provided to Tanknology's Technician, the day of the work via check, credit card or money order for the balance of the contracted work PRIOR TO WORK BEGINNING. 6.0 TERMS: For customers with an account in good standing with TANKNOLOGY we will submit an invoice to the Purchaser in the full amount of this contract. Payment in full is due Net 30 Days subject to 2%per month late payment fee. For customers with current MSA's terms of agreement will be followed.
