Contingency Plan incomplete. <br /> The Contingency Plan for this facility did not include descriptions of <br /> N emergency response to fire or explosion, a list of emergency equipment at <br /> the facility, or the name, address, and phone number of the facility <br /> Emerge cCoordinator. <br /> Retained the services of Condor Earth Technologies, who are in the process <br /> R of drafting the Hazardous Waste Contingency Plan. Will provide appropriate <br /> updates in a follow-up letter by July 29, 2011. <br /> 47 RV 47. No emergency coordinator. <br /> N No Emergency Coordinator has been identified for this facility. <br /> Emergency coordinator is Chief Plant Operator, Fermin Garcia; contact <br /> information is listed on (draft) 1st page of Contingency Plan and all HMMP <br /> R documents for the Main and Tertiary Plants. The Final Contingency Plan <br /> will be forwarded when complete. Will provide follow-up letter by July 29, <br /> 2011. <br /> 23 23. Failed to (1) Drain of free-flowing oil, (2) Reclaim metal, (3) Close <br /> V container w/initial date of accumulation, Label "drained used oil filters'; (4) <br /> Store <1 ton/< 1 ear, 5 Retain bill of lading for filters for 3 years. <br /> N Bills of lading for the recycling of used oil filters for the last three years were <br /> not found on site. <br /> R Bills of Lading for last 3 years are being researched and located. Will <br /> provide follow-up letter by July 29, 2011. <br /> 35 V 35. Failed to conduct daily inspections of HW tanks. <br /> N Hazardous waste tanks on site are being inspected weekly according to Ms. <br /> Lan ston-Kenne . <br /> Operations and Maintenance Divisions have instituted Weekly inspections <br /> R using updated inspection logs that have been provided to them. Daily <br /> Hazardous waste tank inspection forms have been re instituted. Daily and <br /> weekly inspection logs forms are attached. (See Attachment 4) <br /> 49 V 49. Personnel Training Records incomplete. <br /> N A written training program and training records were not available on site. <br /> Attached are available HazMat/HazWaste/EmergencyResponse training <br /> records. The necessary updates and improvements to the current <br /> R HazMat/HazWaste/EmergencyResponse Training Plan are underway. A <br /> written Training program will be submitted to SJEHD no later than July 29, <br /> 2011 in follow-up letter. (See Attachment 5) <br /> 59 V 59. Failed to send generator manifests copies to the Department within 30 <br /> days. <br /> N Manifest numbers 003338193JJK (7/30/09) and 006396849JJK (12/30/10) <br /> were found on site at the facility but were not on record with the DTSC. <br /> R Manifest number 003338193JJK (7/30/09) was sent to DTSC on June 27, <br />
