B St., Stockton Program: APSA <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 24. The contact list did not include phone numbers for San Joaquin County Office of Emergency <br /> Services (OES) and the cleanup contractor. The number for San Joaquin County Environmental Health <br /> Department (EHD) is incorrect. The SPCC Plan must include a contact list and phone numbers for the <br /> facility response coordinator, National Response Center, cleanup contractors with whom you have an <br /> agreement for response, and all appropriate federal, state, and local agencies who must be contacted in <br /> case of a discharge. Immediately update the contact list to include the most current version of the <br /> required information. <br /> 48. Integrity testing was not available on site for the aboveground petroleum tanks and is not addressed <br /> in the SPCC Plan. The facility shall test or inspect each aboveground container for integrity on a regular <br /> schedule and whenever you make material repairs. Examples of these integrity tests include, but are <br /> not limited to: visual inspection, hydrostatic testing, radiographic testing, ultrasonic testing, acoustic <br /> emissions testing, or other systems of non-destructive testing. Comparison records and other records of <br /> inspections and tests must be maintained on site with the SPCC Plan. Immediately conduct all <br /> necessary integrity testing and maintain records on site as required. <br /> 50. Liquid level sensing devices have not been tested. Procedures and frequency of testing for these <br /> devices was not addressed in the SPCC plan. Liquid level sensing devices must be installed in <br /> accordance with 40 CFR 112.8 and shall be regularly tested to ensure proper operation. Immediately <br /> conduct all necessary testing of liquid level sensing devices. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by August 1, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Provided: Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: ReceivJ9 <br /> ed y: Title: <br /> c� Ll <br /> EHD 23-02- 03 08/10/10 CONTINUATION FORM <br />
