The Spill Prevention, Control, and <br />Countermeasure (SPCC) Plan shall designate a person at the facility who is accountable for discharge prevention <br />and who reports to facility management. Immediately designate a person to be accountable for discharge prevention <br />and update the SPCC Plan to include this information. <br />This is a repeat violation, Class II. <br />621 <br />CFR 112.7(f)(3) Failed to schedule and conduct discharge prevention briefings at least annually. <br />Discharge prevention briefings are not scheduled at least once a year. The most recent discharge briefings were <br />held in 2015. Discharge prevention briefings for oil handling personnel must be scheduled and conducted at least <br />once a year to assure adequate understanding of the SPCC Plan for that facility. Such briefings must highlight and <br />describe known discharges or failures, malfunctioning components, and any recently developed precautionary <br />measures. Immediately schedule and conduct a discharge prevention briefing, ensure that they are scheduled and <br />conducted at least once a year. <br />This is a repeat violation, Class Il. <br />622 <br />CFR 112.7(f)(3) Failure to conduct spill prevention briefing for oil -handling personnel. <br />Discharge prevention briefings are not scheduled at least once a year. Discharge prevention briefings for oil handling personnel must be scheduled and conducted at least <br />once a year to assure adequate understanding of the SPCC Plan for that facility. Immediately schedule and conduct a discharge prevention briefing, ensure that they are scheduled and <br />conducted at least once a year. <br />This is a repeat violation, Class ll. <br />701 <br />CFR 112.8(b)(1) Failure to ensure drainage from diked areas is restrained by valves or alternative method. <br />The secondary containment system for the gasoline and diesel tanks is a concrete system with no visible cracks or <br />openings. There were several inches of rain over the past week. Rain water should have been accumulated in the <br />secondary containment. There was no liquid accumulated in the secondary containment. According to Steve <br />Gannon, no liquid has been pumped out, or released from the secondary containment. The facility must restrain <br />drainage from diked storage areas by valves to prevent a discharge into the drainage system. Immediately provide <br />secondary containment for the gasoline and diesel tanks sufficient to restrain drainage from diked storage areas and <br />to contain the entire capacity of the largest single container and sufficient freeboard to contain precipitation. <br />This is a Class II violation. <br />Page 6 of 9 <br />
