(Horizon) is forwarding the <br /> enclosed well permit applications for the installation of a total of seven wells related to the <br /> subject site. The installation of on-site groundwater monitoring well (MW-9) and the three <br /> off-site groundwater monitoring wells (MW-10, MW-11 and MW-12) has been approved by <br /> the San Joaquin County Environmental Health Department (SJCEHD) in their review letter <br /> dated December 13, 2007. Three on-site vapor extraction wells (VW-3, VW-4 and VW-5) <br /> have been proposed in the Letter Work Plan for Additional Vapor Extraction Well <br /> Installation date February 12, 2008 currently under review by the SJCEHD. A copy of the <br /> City of Stockton Public Works Department Encroachment Permit No. 71781 for the work <br /> planned in the city right-of-way is enclosed as part of the off-site wells Permit Application. <br /> Also enclosed are the Licensed Contractors Declaration form completed by All Well <br /> Abandonment (drilling contractor), typical well construction diagrams for each group of <br /> wells, a site map, and a check for $766 to cover the permit fees and inspection fees for two <br /> separate permits at the two locations of the work. The fieldwork is tentatively scheduled for <br /> the week of March 21, 2008.
