(HE) and has the following comments.
In response to EHD letter dated September 25, 2007, this WP was submitted as an addendum to
the Letter fd'ork plan for Additional Subsurface Investigation (WP) dated May 17, 2007 which
proposed installation of wells MW-9 and MW-10 to evaluate the vertical and lateral extent of the
plume.
In the WP, HE proposes two additional monitoring wells MW-1 I and MW-12 located north and
south of well MW-6 to further delineate the location and geometry of the groundwater plume.
EHD approves the proposed installation of these two monitoring wells in addition to MW-9 and
MW-10. The construction of MW-11 and MW-12 should be similar to MW-6 with screen
intervals that include the sand body at approximately 40 to 60 feet below surface grade.
Submit to the EHD a well permit application and $89.00 permit fee, and $294.00 inspection fee
for each parcel drilled and an encroachment permit for the borings in the public right of way.
Once the well permits have been approved, notify EHD of the proposed schedule for the work.
The EHD requires a minimum 48-hour notice prior to initiation of any field work on the site. The
EHD directs that the proposed work be implemented by March 28, 2008.
Please contact Frank Girardi at(209) 953-7868 if you have any questions.
Donna Heran, REHS, Director
Environmental Health Department
i
Frank Girardi, Senior REHS Nuel C.
