August 28, 2013 <br />Mr. Johnny Yoakum <br />San Joaquin County <br />Environmental Health Department <br />1868 Hazelton Avenue <br />Stockton, CA 95205-6232 <br />r <br />RE: MONITORING WELL INSTALLATION APPLICATION <br />WELL INSTALLATION AT FIELD MAINTENANCE SHOP 24 <br />8020 AIRPORT WAY, STOCKTON, CA <br />Dear Mr. Yoakum, <br />RECEIVED <br />AUG 2 9 2013 <br />ENVIRpNMENT <br />PERMITjSECES <br />Enclosed, please find a Well and Boring Permit Application for the installation of a groundwater <br />monitoring well at FMS 24, 8020 Airport Way, Stockton, CA. Also enclosed is a check for the <br />$500 permitting fee (per your conversation with OTIE). <br />As you will see, and as with previous OTIE permits, the billing party is the California Army <br />National Guard (CA ARNG). The CA ARNG asked American Integrated Services to cut the <br />check since it would take too long to get one from the State Military Department, and the timing <br />to install and sample this well is short. The well construction details (attached) were specified <br />by OTIE to match the previously installed wells, and their plans are provided in Work Plan for <br />Implementing Interim Remedial Action by in -Situ Chemical Oxidation Treatment, prepared <br />August 13, 2013. The well installation and its proposed location was directed by the Central <br />Valley Regional Water Quality Control Board in their Monitoring and Reporting Program issued <br />for this site. The proposed location is shown on the attached figure. <br />It is critical that installation of this well ocurr on Monday, September 16, 2013; therefore, we <br />respectfully request that the County process the permit in time to meet this deadline. The well is <br />required by the Water Board for remedial activities that OTIE must start by Septemeber 23, <br />2013. <br />Should you have any questions regarding this application.
