Type of incident: Spill ®' Fire Q Explosion <br /> 2. Location of incident: RFI line in back lot parking area <br /> 3. Time/Date incident occurred: 17 APR 2014, 1115 <br /> 4. Name and approximate quantity of hazardous/toxic material involved: <br /> FP <br /> 8 Diesel Fuel 20-30 Gallons <br /> 5. Approximate extent of area contaminated:10X12 foot area in soil, depth unknown <br /> 6. Number and cause of in'uries: <br /> ZERO <br /> 7a. Was the facility evacuated? Yes 0 No <br /> 7b. Was surrounding area evacuated? Yes 0 No <br /> 8. Brief description of incident: <br /> M978 fuel truck,was seeping fuel from the filter canister housing drain. Length of time of <br /> seeping is unknown, soil contamination was spotted by SSG Koenig who then advised <br /> MSG Patchen, at which time he contacted me to investigate. I made the determination that <br /> it was a reportable spill and the initiated spill response. <br />
