The 91 STP sump sensor was replaced April 21, 2014 and a permit was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHUS policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the 91 STP sump sensor. A "Failure to Obtain a Permit" penalty applies. This is a Class II violation. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The 87 sump sensor was located on top of the saddle and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation.
