Tracy <br />December 31, 2014 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS 11, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />317 <br />HSC 25290.1, 25290.2, 25291 Secondary containment not tight. <br />The 12/13/2013 secondary containment test results indicate a failure at the 91 STP sump. A repair permit was <br />obtained from this office March 24th, 2014. On June 12, 2014 inspector Thuy Tran noted she had spoke with Wayne <br />Henderson, HMC. It was noted repairs had been made but the 91 STP sump was still leaking. He was unable to <br />repair. On August 31, 2014 BZ maintenance sent an email to Michelle Henry stating" the electrical penetration that was <br />previously repaired is the location of the leak. Because of the angle of the electrical line, the boot seam is no flush one <br />side to the other or the wall. There is a lot of sealant around it, but there was still a hole on the underside big enough <br />for my technicians finger." BZ was notified they were not able to repair the UST system due to the lack of the correct <br />contractors license. <br />During the monitoring certification today, December 31, 2014, Tony Fontana retested the 91 STP sump using an Incon <br />device. The 91 STP sump passed a hydrostatic test. Immediately submit a statement to the EHD explaining how the <br />91 STP sump was fixed. <br />All secondary containment for the UST system must be tight. Immediately have a properly licensed, trained, and <br />certified contractor repair or replace the failed component under permit and inspection of the EHD. <br />This is a Class II violation. <br />319 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the 87 sump. If water could enter into the secondary containment by precipitation or infiltration, <br />it must be removed and disposed of properly. The service technician removed 1-3g of liquid from the 87 sump. <br />Ensure that all sumps and annular spaces are maintained free of liquid. <br />This is a Class II violation. <br />2030 <br />CCR 2661(b) Requirements for repairing Underground Storage Tank System: <br />A cold start was performed at this site on February 27, 2014. A repair permit was issued by this office on March 24, <br />2014 to repair the 91 STP and test functionality of the monitoring system after a power outage. The permit states <br />inspections for emergency work must be scheduled within five days of permit approval. The functionality of the <br />monitoring system was not conducted until today, December 31, 2014. Take all necessary precautions to ensure <br />permit conditions are met and followed through with. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by{YOffE, 30 days from now] . <br />yuwd,111 31, 2o1 <br />Consent to perform the inspection, take photos and collect samples was given by Raj Deol, store manager. This <br />facility's designated operator is Tony Fontana (ICC expiration date:5/2016 ) .
