Waste vacuum contaminated with chromium, Page 2, 28i <br />(i) Waste carbon contaminated with tetrachloroethylene (3 drums), <br />Shipped on 2125103 on Manifest #96894232 <br />(j) One drum contained cement. Considered a hazardous material. This <br />was removed from the hazardous waste storage area. The other <br />container was empty and was also removed from the area. <br />(k) Waste PPE contaminated with MEK (2 of 2), Page 2, 28h <br />(1) Universal waste — Non PCB ballasts, to be shipped 3/27103 <br />(m) This container is a drum with vacuum attachment used to vacuum the <br />adhesive primer spray booth. An accumulation label has been <br />attached to the drum indicating that it will be emptied after each use. <br />(n) Waste Jemstrip solution used to clean adhesive primer paint pots and <br />spray gun equipment (1 of 2), Page 2, 28g <br />(o) Waste methylene chloride/resin, (30 gal. drum), Page 4, 28e <br />All other items on the manifest were lab pack items, regular wastestreams, or <br />intermittent wastestreams. Signed off manifests from the TSDF will be <br />submitted upon receipt. <br />420 Failed to properly label containers of hazardous waste. <br />To the best of my knowledge, all of the "Non-RCRA" labeled hazardous <br />waste containers have been marked to indicate hazardous properties. <br />To the best of my knowledge, all of the small red metal containers have been <br />labeled with a hazardous waste label, hazard label, and accumulation label <br />indicating that they will be emptied daily. <br />#21 Failed to keep containers of hazardous waste closed except when adding or <br />removing hazardous waste. <br />(a) One drum contained cement, and the other drum was empty. Both <br />drums were removed from the hazardous waste storage area. <br />(b) The 55 gallon drum of graphite pre-preg (not film adhesive) was closed <br />and removed from the Blue Room. <br />(c) The bung on the 30 gal. acetone/sulfuric acid waste drum was replaced <br />and corrected on-site. <br />#27 Failed to keep signed copy for 3 years. <br />A signed copy of Manifest #20828299 from the TSDF, dated 08/13/01 is <br />attached. <br />#49 Personnel training records incomplete. <br />
