SUMMARY OF VIOLATIONS CLASS I, CLASS II, or MINOR -Notice to Comply) Onsite to conduct a routine hazardous waste inspection. No hazardous waste disposal records were readily available for review at the time of inspection. Mr. Jimmy Mozingo stated that no hazardous waste has been transported for disposal since the last routine hazardous waste inspection. Mr. Mozingo also stated that all truck maintenance is being conducted at an offsite location and no hazardous waste is routinely being generated at this facility. All universal waste fluorescent light bulbs are also being transported to an offsite facility for consolidation and disposal. Class II Violations: 101, 305, 407, 605, 703 101. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit evidence to the EHD by June 6, 2014- A form has been provided. 305. A spill pallet outside the warehouse (near the hazardous material storage area) was observed with standing liquid in the bottom. All facility communications and alarm systems, fire protection equipment, spill control equipment, and decontamination equipment, shall be tested and maintained as necessary to assure its proper operation in time of emergency. Spill containment shall be maintained clean and dry at all times so as to detect a leak at the earliest opportunity. Immediately remove and properly dispose of all hazardous waste from spill containment, and 7tain all spill containment clean and dry at all times. ALL END STAFF TIME ASSOCIATED WITH FAILING PLY BY JE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
