i 1 <br /> 7. Provide information on disposal of hazardous material (s) <br /> used at the site including disposal location, frequency of <br /> disposal, quantities disposed, and whether the waste <br /> remains onsite. If not, the time period the item was on <br /> site, when it was removed, and by whom. <br /> 8. Identify all leaks, spills, releases or threats of releases <br /> of any hazardous materials that occurred at or from the <br /> site. Identify any operational methods that would allow <br /> for drips, and/or spills of hazardous materials to the <br /> floor of the buildings an/or onto the ground. Also <br /> identify operational methods that would allow hazardous <br /> materials to be routed into a collection sump or sewer. <br /> Include in this identification: <br /> a. When and how such releases occurred; <br /> b. What hazardous materials were released; <br /> C. What amount of each such materials was so released; <br /> and <br /> d. Where such releases occurred (identify on site <br /> diagram) . <br /> 9. Identify any past uses of onsite ponds or ditches (i.e. , <br /> runoff collection, etc. ) . <br /> 10. Provide all existing information from the sampling of any <br /> air, soil, and/or water (ground or surface) on or about the <br /> site as detailed below: <br /> a. Summary of sampling efforts (federal, state, local, <br /> responsible parties) including sampling objectives, <br /> locations, and results; <br /> b. Data quality/adequacy evaluation (qualitative) ; <br /> C. Any cleanup activities taken at the site including <br /> past cleanup sampling data; and <br /> d. Standards and protocol followed for sampling/analysis <br /> activities (can reference sampling plan or other <br /> document) . <br /> 11. Provide copies of any/all documents to the business <br /> operation of the facility. <br />
