d. Any and all areas in which hazardous materials were <br /> used, stored, and/or disposed of; <br /> e. Ground water monitoring wells, ground water supply <br /> wells, vadose zone wells, storm water drainage system, <br /> and sanitary sewer system, including septic tank(s) <br /> and subsurface disposal system(s) . <br /> 5. Include the following information as an attachment to the <br /> site diagram: <br /> a. A detailed description of the type of business or <br /> operation conducted in each area and/or structure; <br /> b. A description of each type of hazardous material <br /> stored at the site, where it was stored, the quantity <br /> and its use; and <br /> C. A description of the use(s) of any onsite sump, <br /> collection system, sanitary sewer system, septic <br /> tank(s) , and/or subsurface disposal system(s) . <br /> 6. Did you ever transport to the site or use, generate, store, <br /> treat, or dispose, or otherwise handle at the site any <br /> hazardous materials? If the answer to the preceding <br /> question is anything but an unqualified "no" , identify: <br /> a. List the name, chemical composition, and <br /> characteristics of concern such as toxicity, <br /> persistence, and flammability for each constituent. <br /> b. For each hazardous material and constituent, describe <br /> the following: <br /> 1. Physical state (e.g. solid, liquid) of each <br /> hazardous material handled; <br /> 2 . How each hazardous material or waste was used, <br /> generated, stored, treated, transported, <br /> disposed, or otherwise handled; <br /> 3 . When each such hazardous materials or waste was <br /> used, generated, stored, disposed, or transported <br /> or otherwise handled; <br /> 4 . Where each such hazardous material or waste was <br /> used; and <br /> 5. The quantity of each such hazardous material or <br /> waste used, generated, stored, treated, <br /> transported, disposed, or otherwise handled. <br />
