Training records for CA Haz Waste Handler is missing description of course information. Training <br /> records shall be maintained on site until closure of the facility and shall include: <br /> - name &job title of each employee, - description of training to be given to each position <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties, - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program. <br /> Submit a description of training course for CA Haz Waste Handler. Ensure a copy of training description <br /> is kept with the training log onsite. <br /> 60, 61, & 62. Manifest number 008142851 JJK shipped 05/04/11 containing 150 pounds of oily solids <br /> was found without a signed off copy from the destination facility. Hazardous waste generators shall <br /> retain copies of all manifests signed off by the destination facility on site for three years and have them <br /> readily available for review. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 35 <br /> days of the date the waste was accepted by the initial transporter, the generator shall contact the <br /> transporter and/or the owner or operator of the designated facility to determine the status of the waste. <br /> If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br /> that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br /> that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br /> explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports <br /> shall be submitted to: DTSC Report Repository, Generator Information Services Section <br /> P.O. Box 806, Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report <br /> submitted to DTSC. <br /> Submit to SJC EHD within 30 days (09/05/11): <br /> 1. one copy of the "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> 3. requested/ supporting documentation <br /> Notes: - Provided hazardous waste labeling instructions <br /> - In 2010 year, facility generated approximately 6195 gallons (24.78 Tons) of used oil, 525 gallons (2.1 <br /> Tons) of used antifreeze and 40 pounds (0.02 Tons) of brake shavings = 26.9 Tons of hazardous waste. <br /> - Current free training schedule printed on site by service mgr. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY T E ATf HD'S CURRENT HOURLY RATE. <br /> EHD Inspect : Received B Title: _ <br /> /Lt �j�jGa <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
