Training records for CA Haz Waste Handler is missing description of course information. Training records shall be maintained on site until closure of the facility and shall include: - name &job title of each employee, - description of training to be given to each position - written job description for each position (job title) listed, including requisite skill, education, qualifications, and duties, - records that training was completed for each employee Immediately prepare and implement a complete hazardous waste management training program. Submit a description of training course for CA Haz Waste Handler. Ensure a copy of training description is kept with the training log onsite. 60, 61, & 62. Manifest number 008142851 JJK shipped 05/04/11 containing 150 pounds of oily solids was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository, Generator Information Services Section P.O. Box 806, Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. Submit to SJC EHD within 30 days (09/05/11): 1. one copy of the "Return to Compliance Certification" form 2. a written corrective action statement for the above violations 3. requested/ supporting documentation Notes: - Provided hazardous waste labeling instructions - In 2010 year, facility generated approximately 6195 gallons (24.78 Tons) of used oil, 525 gallons (2.1 Tons) of used antifreeze and 40 pounds (0.02 Tons) of brake shavings = 26.9 Tons of hazardous waste. - Current free training schedule printed on site by service mgr. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
