0
TVV® ® S H
Response / Follow Up: Fleetwash has established a proper receptacle for drained used oil filters.
This receptacle is labeled "drained used oil filters," is closed, lists the starting accumulation date,
and contains less than one ton of drained used oil filters. Fleetwash has established a service
schedule with Safety Kleen to have drained used oil filters removed every twelve months or
more frequently, if required. Bills of lading will be stored onsite. Photo in appendix.
With regard to disposal records for the last three years, Fleetwash has been unable to locate
complete records due to a recent management change. Miscellaneous records for oil and filter
changes performed by outside vendors offsite have been located and copies of those records have
been placed in a newly created oil filter disposal file.
Line Item 27: Failed to keep containers of hazardous waste closed except when adding or
removing hazardous waste.
Response / Follow -Up: The used oil in the container in question has been removed by Safety
Kleen with the used oil mentioned above and a manifest has been provided. That manifest will
be kept on file. As described above, a used oil storage area has been established. The containers
used in this storage area have lids. Photo attached.
Line Item 52: No emergency coordinator; no modified contingency plan.
Response / Follow -Up: Ralph Colonna has been designated as the onsite / on-call emergency
coordinator. Jim DiCarlo has been designated as the alternate emergency coordinator. The
required emergency coordinator and modified contingency plan information has been posted 1)
near all phones, and 2) near the time -clock and right to know center. Photo in appendix.
Line Items 60, 63, and 85: Failed to keep signed copy for three years (60). Failed to keep a
consolidated manifest for three years (63). Failed to keep records of each shipment of universal
waste according to this section (85).
Response / Follow -Up: While unable to locate all bills of lading and manifests, those which are
available have been compiled and placed in a consolidated manifest file. Those records are in the
attached appendix. Fleetwash will properly maintain the newly created consolidated manifest
file.
