Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />May 22, 2014. <br />122, 123, & 124. Manifest numbers 002920044SKS, 003324261 SKS, 003629111 SKS, 003824091 SKS, <br />003824308SKS were found without a signed off copy from the destination facility. Hazardous waste <br />generators shall retain copies of all manifests signed off by the destination facility on site for three years <br />and have them readily available for review. If the generator did not receive a copy of the manifest with <br />the handwritten signature of the owner or operator of the facility to which the generator's waste was <br />submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br />shall contact the transporter and/or the owner or operator of the designated facility to determine the <br />status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within <br />45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br />Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br />generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br />Exception reports shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate copies of the missing manifests or prepare and submit an exception report to DTSC. <br />By May 22, 2014, submit to the EHD a copy of the signed off copy of the manifests or a copy of the <br />exception report submitted to DTSC. <br />202. Oily sludge generated at this facility is being transported for disposal under the consolidated <br />manifesting procedure by Safety-Kleen Systems. Inc. On multiple occasions this waste stream, which is <br />being classified as "Non-RCRA Hazardous Waste Liquids (oily sludge water)", was transported to a <br />disposal facility under the consolidated manifesting procedure. No person shall knowingly offer ineligible <br />waste streams for transportation under the consolidated manifesting program. If generator does not <br />generate more than 1,000 kilograms of waste per calendar month, then wastes listed in HSC 25160(c) <br />(1) and HSC 25160(c)(2) may be manifested under the consolidated manifesting program.
