Broadway, #11 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> This is a Follow-Up Hazardous Waste Inspection billable at current EHD hourly rate. The following is a <br /> Minor Violations - Notice to Comply: #8, and the following are Class ll Repeat Violations: #20,23,3. <br /> 8. This facility does not have a valid EPA ID number to manage hazardous waste. <br /> -- A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, <br /> hazardous waste without an EPA ID number. <br /> * Immediately obtain an EPA ID number to manage hazardous waste and submit evidence to the EHD <br /> by July 7, 2011. A form has been provided. <br /> 20. Used Oil tank missing start date of accumulation. <br /> -- All hazardous waste tanks shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. accumulation start date <br /> * Immediately label these tanks and ensure that all hazardous waste tanks are marked with all the <br /> required information. Violation was corrected on site. (Last used oil disposal receipt was dated May <br /> 2011. The tank only had a few inches of used oil) <br /> 23. Observed two open half-barrels of drained oil and diesel fuel filters. One 55-gallon drum of about 10 <br /> used oil filters is missing the date of accumulation. <br /> -- Used oil filters must be stored in a closed, rainproof container, labeled as "drained used oil filters," <br /> and marked with an accumulation start date. <br /> * Immediately place the used oil and diesel fuel filters in a closed rainproof container, and ensure that all <br /> used oil filters are managed in this way. Submit evidence of correction to the EHD by July 7, 2011. <br /> Violation of labeling was corrected on site. (Last drained oil filters disposal was in May 2011.) <br /> NOTES <br /> Observed during this inspection the facility meeting return to compliance requirements listed in the <br /> Official Inspection Report dated March 20, 2009: <br /> (12) Facility has minimized release of hazardous waste to the environment. <br /> (20,21) No hazardous waste containers on site.
