Broadway, #11 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> (32,33) Labeling of drained oil filter container was corrected on site (See #23 above). <br /> (90) Emergency Coordinator and Contingency Plan information posted. <br /> (91) No empty hazardous waste or hazardous material greater than 5-gallon capacity on site. Old <br /> empties appeared to have been converted to garbage cans. <br /> Discussed with generator Universal Waste management requirements including aerosol cans, lamps, <br /> alkaline batteries, mercury containing switches, CRT, electronic devices, etc. in a closed container with a <br /> label identifying the content and the start date of accumulation. Universal Wastes must be managed <br /> within 1 year of accumulation, and the disposal records maintained on site for at least 3 years. <br /> A copy of Auto Repair Best Management Practice factsheet, CUPA class schedule, blank hazardous <br /> waste labels, hazardous waste labeling factsheet, have been provided to generator during this <br /> inspection. <br /> Submit the following to EHD by: July 7, 2011 <br /> 1) A completed copy of the (provided) Return to Compliance Certification (please keep original for <br /> your records). <br /> 2) A written statement how each violation was corrected <br /> 3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THI F CILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD ect Received By: Title: <br /> Iv/ CONTINUATION FORM <br /> EIZD 23-6-11163 aev 08/10/10 <br />
