The EHD has written the <br /> complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov. Be sure to include <br /> your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant <br /> activities and required fields. <br /> Note.- <br /> The <br /> ote:The following issues are first time violations that were corrected during the inspection. <br /> 103. No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of <br /> fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department <br /> unless the facility has a direct alarm. During the inspection, the provided form was completed and posted. <br /> Continue to keep the required information posted by the facility phone. <br /> 403. Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. <br /> The unlabeled 250 gallon tote of hazardous waste used oil stored outside was left open. Inside the shop were twc <br /> five gallon unlabeled open buckets of hazardous waste contaminated soil that Brad Sporleder confirmed are <br /> hazardous waste. All hazardous waste containers shall be closed at all times except when adding or removing <br /> waste. During the inspection, these containers were closed. Continue to ensure all hazardous waste containers <br /> are closed when not adding or removing waste. <br /> 605. Failed to completely label containers or tanks of hazardous waste. <br /> The unlabeled 250 gallon tote of hazardous waste used oil stored outside was left open. Inside the shop were twc <br /> five gallon unlabeled open buckets of hazardous waste contaminated soil that Brad Sporleder confirmed are <br /> hazardous waste. The 55 gallon unlabeled drum of hazardous waste hydraulic oil lacked the required hazardous <br /> waste information on it. All hazardous waste containers shall be marked with the following information: <br /> - the words "Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> - composition (contents) <br /> - accumulation start date <br /> Page 6 of 7 <br />
