A copy of each HW disposal receipt was not available for inspection. <br />-- Facilities shall retain a copy of the signed copy of the Uniform HW manifest, and a copy of the <br />consolidated HW manifest, for a minimum of 3 years available for inspection. <br />* Submit a copy of last 3 years of all disposal receipts to EHD. Begin retaining a copy of all disposal <br />receipts and signed Uniform HW manifests for 3 years on site available for inspection. <br />NOTES (The following are not violations) <br />7. Observed oil in 3 secondary containments, and metal shavings by lathes. Explained to generator <br />that it is their responsibility to make a HW determination of a waste and manage it accordingly, or the <br />generator could determine the waste as a HW and dispose it in accordance to California Code of <br />Regulations (CCR) Title 22. If the waste is determined to be a non -HW, the generator shall submit <br />evidence (i.e. third party laboratory testing results) supporting such a determination to EHD. Metal <br />particle that is less than 100 microns in diameter and is hazardous must be managed as HW; otherwise, <br />it can be recycled as scrap metal. Observed oil from secondary containment disposed to the used oil <br />container and metal shavings moved to scrap metal recycling bin. Corrected on site. <br />- Provided to generator a copy of CUPA class schedule and Best Management Practice (BMP) auto <br />repair shops. <br />- Submit the following to EHD by September 14, 2010: <br />1) A copy of the completed Return to Compliance (provided), <br />2) A written statement how each of the above violation was corrected, <br />3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br />4) Updated Form 5021 (provided during this inspection) <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD In clor: <br />Receive <br />Title: <br />EHD 23-02-003 Rev 08110/10 CONTINUATION FORM <br />
