V. STATEMENT OF FACTS: <br /> 7-20-89 <br /> PHS-EHD performed a Hazardous Waste Generator inspection of the <br /> facility as requested in complaint #89-1429 from the County Bureau <br /> of Fire Prevention. (SEE EXHIBIT A) <br /> The inspection revealed as many 100 unlabeled 55 gallon barrels. <br /> Mr. Ricketts, General Manager of Aero Nostalgia, was present during <br /> the inspection and identified the unlabeled barrels as containing <br /> various waste oils and solvents. <br /> Mr. Charles Hunt, EHS, presented handouts on obtaining an EPA <br /> number and a registered hazardous waste hauler. A "Notice to Abate" <br /> was issued by Mr. Hunt and signed by Mr. Ricketts and required <br /> removal of the hazardous waste barrels by 8-15-89 . <br /> (SEE EXHIBIT B) <br /> 8-14-89 <br /> Mr. Hunt called Mr. Ricketts to confirm the proper removal of the <br /> hazardous waste barrels. Mr. Ricketts was not in and a message <br /> was left to return the call. <br /> 8-21-89 <br /> Mr. Hunt made contact with Jim Ricketts by phone. Mr. Ricketts <br /> indicated that Telic Engineering, a registered hazardous waste <br /> hauler, had been contracted to remove the waste barrels within 3 <br /> weeks. Mr. Ricketts was instructed to send copies of any receipts <br /> or manifests upon removal of the waste barrels. <br /> 9-25-89 <br /> Charles Hunt made a site visit and noted that the barrels had not <br /> been removed. <br /> 10-13-89 <br /> Charles Hunt, EHS, contacted Jim Ricketts who indicated he had been <br /> attempting to contact Wendall Thompson of Telic Engineering, in <br /> order to get him to remove the barrels from the site. <br /> 3 <br />
