In the <br /> appendix there are ignitables, oxidizers, corrosives (both high and low pH) identified as waste streams in <br /> pharmacies <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br /> waste handling and emergency procedures. Provide proof of training to the EHD for employees whose <br /> responsibilities include hazardous waste. <br /> This is a Class II violation. <br /> 203 HSC 25189.5(c) Transported or caused transportation of hazardous waste to an unauthorized point. <br /> At time of inspection: <br /> St. Joesphs' hospital is using Inmar, Med-Turn Inc. as their reverse distribution company. According to Jacquelyn <br /> Cituk, Director of Pharmacy, pharmaceuticals which are expired or are expiring are put into a box in the pharmacy. <br /> A representative from Inmar comes in, segregates the controlled substances, and takes all the"unusable <br /> pharmaceuticals"to the the Med-Turn location in Fort Worth, Texas. RCRA pharmaceuticals are not segregated <br /> from non-RCRA pharmaceuticals. It is the Pharmacists understanding from the Inmar representative on site that <br /> only controlled substances are eligible for credit.
