San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Far: (209) 468-3433 Web:www.sjoov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ST JOSEPHS HOSPITAL 1 1800 N CALIFORNIA ST, STOCKTON April 21, 2017 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> At the time of inspection: <br /> Hazardous waste staining mixture in the Healthcare Clinical Laboratory is poured into a container in two sinks. <br /> Both sinks appeared to be stained from drips, splashes, over pours, etc. In each sink there is an unplugged drain. <br /> Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment <br /> Immediately implement a procedure to minimize the release of the hazardous waste and begin managing according <br /> to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this <br /> waste was managed. <br /> This is a Class II violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> At time of inspection: <br /> One 55g drum of viral disinfectant <br /> One 55g drum of methanol, acetone <br /> were seen open. <br /> All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br /> close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> This is a Class II violation. <br /> 603 CCR 66262.34(e)(1)(C) Failed to mark initial accumulation start date on satellite containers. <br /> At time of inspection: <br /> One 1 g container in the main lab did not have an initial start accumulation date. <br /> One 1 g container in the main lab had an incomplete initial start accumulation date (date, month, without a year). <br /> Several 1-g containers in the main hazardous waste storage area had dates marked in pen on the plastic that wiped <br /> off or were illegible. <br /> Several 1 g containers in the main hazardous waste storage area did not have an initial start accumulation date. <br /> Several 1 g containers had incomplete initial start accumulation dates(date, month, without a year). <br /> The initial date of waste accumulation shall be clearly marked and visible for inspection on each container used for <br /> satellite accumulation of hazardous waste.
