Confined space contractors are expected to have an appropriate <br />written confined space entry program and must provide an entry permit, attendant(s), air -monitoring <br />equipment, and an appropriate rescue process in accordance with OSHA standards. Documentation of <br />training and certification to use appropriate equipment is also required. It is the DC's responsibility to <br />inform contractors about the hazards of the space in which they will be working and that compliance <br />with this policy is mandatory. All information about the hazards of that space will be shared with the <br />contractor prior to their entry. <br />Contractors working in confined spaces should be trained to this standard prior to coming to the <br />Stockton DC to perform work. The Contractor will be asked to provide a copy of their written confined <br />space program and documentation of training for the people working in the confined space. The <br />Contractor must inform the Stockton DC of the confined space program that the contractor will follow. <br />Contractors will ensure that confined space openings are protected from by-standers accidentally <br />walking into the opening by posting a guard to prevent accidents. <br />5.1 Entry Permit — A permit, signed by the entry supervisor designated by the contractor will <br />verify that pre -entry preparations have been completed and that the space is safe to enter. The <br />permit must be posted at entrances prior to the entrants entering the permit space. <br />The duration of entry permits must not exceed the time required to complete an assignment. <br />Also, the entry supervisor must terminate entry and cancel permits when an assignment has <br />been completed or when new conditions exist. New conditions must be noted on the canceled <br />permit and used in revising the permit space program. The standard also requires the employer <br />to keep all canceled entry permits for at least one year. <br />The contractors' entry permits must include the following information: <br />• Test results, <br />• Tester's initials or signature, <br />• Name and signature of supervisor who authorizes entry, <br />• Name of permit space to be entered, authorized entrants, eligible attendants, and <br />individuals authorized to be entry supervisors, <br />• Purpose of entry and known space hazards, <br />• Measures, to be taken, to isolate permit spaces and to eliminate or control space <br />hazards i.e., locking out and tagging of equipment and procedures for purging, <br />making inert, ventilation, and flushing permit spaces, <br />• Name and telephone numbers of rescue and emergency services, <br />• Date and authorized duration of entry, <br />• Acceptable entry conditions, <br />• Communication procedures and equipment to maintain contact during entry, <br />• Additional permits, such as for hot work, that have been issued to authorize work in <br />the permit space, <br />• Special equipment and procedures, including personal protective equipment and <br />alarm systems, and <br />
