This includes placing a part of the body, such as a hand, in a
confined space. Any work in the confined space will only be conducted using authorized contractors
and approved beforehand by Cary Childers or Frank Cereghini.
4.1 Rules for Prevention of Unauthorized Entry. All identified confined spaces will be labeled
with wording such as "Confined Space - Unauthorized Personnel Stay Out/Do not Enter". All
associates working in areas where confined spaces are identified will be trained to not enter
confined spaces. In the event it is necessary for work to be performed within the space, an
authorized supervisor (John Copenhaver) will direct authorized contractors.
In the event a permit is required to perform work, a copy of the OSHA approved permits issued and
used by the authorized contractor must be kept on file at the Stockton DC for a minimum of one year,
(12 rolling months).
Confined space awareness training is required upon initial assessment, during new Associate
orientation or when previously identified spaces are changed or modified. If associates demonstrate
during the interview process on audits, inspections and safety observations that additional training is
required, we will evaluate the effectiveness of our current program and retrain associates.
This policy will be reviewed and updated annually by John Copenhaver. The review will use system
audits, incident reports and other information to evaluate its effectiveness. All findings, deficiencies,
and recommendations for program improvement will be documented and assigned to the HSIT TEAM
for corrections by a specified date. All corrective actions will be noted in the review history at the end of
this document. If no changes are required that will be noted in the review history.
