Manifest numbers: 005105826SKS (10/29/15), 005895454SKS (5/12/2017), and 005863025SKS (5/12/2017) were found without a signed off copy from the destination facility. Manifest number 006099834SKS (5/19/2017) was not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator’s waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a Class II violation. 113 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records for 2016 were not found on site. In addition, records indicate that there was only one pickup event of hazardous waste for 2017 (5/12/2017) and two for 2018 (2/8/2017 and 2/13/2018). Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies of all hazardous waste disposal records (including universal waste) for 2016, 2017 and 2018 to the EHD. This is a minor violation. 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. In storage area one (area closest to the office): -Two black 55 gallon steel drums containing oily debris/oily containers (both over 1/2 full) were observed not completely shut. In storage area two (near the lockers): -Two black 55 gallon drums containing oily debris/oily containers were observed overfilling. -One black 55 gallon drum containing oily debris/oily containers was observed not completely shut. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. This is a minor violation.
