Provide the City with a spill report stating why the discharge occurred, a description of the <br /> substance spilled,estimated start date of the discharge, corrective actions taken upon <br /> notification, and a spill prevention plan to prevent future discharges. <br /> 5. Notify the City upon completion of corrective measures for an inspection and approval of the <br /> system. <br /> We urge you to address these storm water issues immediately.
