Report completed on April 21, 2011. Checklist left on site at <br /> time of inspection. Violations # 60, 61, 62, and 81 have been added. <br /> Class II violations: (7, 20, 23, 82, 84) <br /> 7. Metal fines generated from cutting and grinding are being recycled as scrap metal without first <br /> determining particle size or hazardous properties. Any person who generates a waste shall determine if <br /> the waste is a hazardous waste. Immediately stop recycling metal fines, make a hazardous waste <br /> determination for the cutting and grinding waste, and manage it according the Title 22 hazardous waste <br /> regulations. Particles 100 microns or smaller must be handled as hazardous waste if the metal is <br /> determined to be a hazardous waste. Submit a statement and supporting documentation explaining how <br /> this waste was managed by May 21, 2011. <br /> REPEAT VIOLATION <br /> 20. A 55 gallon red steel drum of absorbent and oil was observed in the shop without a hazardous <br /> property marked. All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Label was corrected on site. <br /> REPEAT VIOLATION <br /> 23. A 55 gallon black steel drum of drained used oil and gasoline filters was observed in yard north of <br /> the shop without an accumulation start date listed. Used oil filters must be stored in a closed, rainproof <br /> container, labeled as "drained used oil filters," and marked with an accumulation start date. The <br /> accumulation start date was written on at the time of inspection. <br /> REPEAT VIOLATION <br /> ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 v 8110110 CONTINUATION FORM <br />
