Charter Way, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,GLASS II,or MINOR-Notice to Comply) <br /> 82. This facility collects CRT waste, which is collected and stored in metal dumpsters in a manner that <br /> does not protect against breakage. A generator shall manage CRTs in a manner that prevents releases <br /> of any CRTs or component to the environment under reasonably foreseeable conditions. CRTs should <br /> be contained in a container or package that is structurally sound, adequate to prevent breakage, and <br /> compatible with the contents of the CRT. A generator shall immediately clean up and place in a <br /> container any CRT that is broken or shows evidence of breakage, leakage, or damage that could cause <br /> release of the CRT glass or other constituents. The containers shall include packing materials if <br /> necessary to prevent breakage during handling, storage, and transportation. Immediately begin <br /> managing these waste CRTs in accordance with Title 22 regulations and ensure that all waste CRTs are <br /> managed this way in the future. <br /> REPEAT VIOLATION <br /> 84. The universal waste fluorescent bulbs were observed with an accumulation start date of March 27, <br /> 2010. A universal waste generator shall accumulate universal waste for no longer than one year from <br /> the date the waste was generated. Immediately dispose of this waste as universal waste and submit a <br /> copy of the disposal record to the EHD by May 21, 2011. <br /> REPEAT VIOLATION <br /> Notice to Comply - Minor violations: (32, 49, 60, 61, 62, 81, 85) <br /> 32. A tank assessment was not available on site for the used oil and used antifreeze tanks. A <br /> hazardous waste generator shall obtain and keep on site a written assessment for each hazardous <br /> waste tank at the facility. This assessment shall be reviewed and certified by an independent, qualified, <br /> professional engineer, registered in California, and shall attest to the tank system's integrity. The <br /> assessment shall determine that the tank system is adequately designed and has sufficient structural <br /> strength and compatibility with the waste stored, and shall be written in accordance with Title 22 <br /> regulations. Immediately obtain a written tank assessment for the used oil tank at this facility in <br /> accordance with Title 22 regulations, maintain a copy on site at the facility, and submit a copy of the <br /> certified assessment to the EHD. <br /> 49. A written training program and training records were not available on site. Facility personnel shall <br /> complete a training program in accordance with Title 22 regulations. This program shall be directed by a <br /> person trained in hazardous waste management procedures and shall include the following: <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02- 3 ev o811oil o CONTINUATION FORM <br />
