The modified contingency plan was not posted.There <br /> must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the <br /> following information must be posted by a phone:the name and phone number of the emergency coordinator; <br /> location of fire extinguishers, spill control equipment, and if present,fire alarm; and the phone number of the fire <br /> department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the <br /> required information by a phone. A form is provided that can be used for this purpose. <br /> This is a minor violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> One uniform manifest for the disposal of absorbent was found on site from 2017. One disposal record for used oil <br /> filters(metal/paper)was observed on site from 2015 and three disposal records were observed on site from 2016 . <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site. Submit the <br /> used absorbent records for 2015,2016, and 2017. Submit the used oil filter records(metal and paper)for 2015 and <br /> 2016. <br /> Manifest numbers 005729809SKS(1/6/2017), 006099602SKS(11-2-2017), 006325840SKS (1/22/2018), <br /> 006513918SKS(4/5/2018), 006513923SKS (4/5/2018), 006513919SKS(4/5/2018), and 006520565SKS <br /> (4/23/2018) were found without a signed off copy from the destination facility. Hazardous waste generators shall <br /> retain copies of all manifests signed off by the destination facility on site for three years and have them readily <br /> available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste <br /> was accepted by the initial transporter,the generator shall submit a legible copy of the missing manifest, with some <br /> indication that the generator has not received confirmation of delivery.
