The fire extinguishers on site had an annual maintenance date of October 25, 2010. One of the <br /> extinguishers had a gauge indicating it needed to be recharged. All facility communications, alarm <br /> systems, fire protection equipment, spill control equipment, and decontamination equipment shall be <br /> tested and maintained as necessary to assure its proper operation in time of emergency. Immediately <br /> have all necessary inspections and service completed for all fire extinguishing equipment at this facility, <br /> and ensure proper maintenance is conducted in the future. <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit proof of correction to the EHD by February 3, 2013. <br /> 703. At the time of inspection, it could not be demonstrated (not necessarily documented) that <br /> employees who handle hazardous waste were properly trained. According to Mr. Ramos, training has <br /> not been conducted in a long time. The generator must ensure that all employees who handle <br /> hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. <br /> Immediately provide training to all employees who handle hazardous waste and submit a copy of the <br /> training records to the EHD by February 3, 2013. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by February 3, 2013 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Provided: Modified Contingency Plan template, Application for California EPA ID number, Sample <br /> hazardous waste label, Hazardous waste labels, Small Business Household Hazardous Waste Facility <br /> brochure, CERS Lead User Authorization form, CERS HMMP flyer, Return to Compliance Certification <br /> form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
