Manifest numbers 000176133JJK from 619108, 001243447JJK from 10123/08, <br /> 001243663JJK from 816/08, 001243673JJK from 8/20/08, 001243684JJK from 9/15/08, 000868988GBF <br /> from 9/18/09, 000869098GBF from 4/7/10 were found without a signed off copy from the destination <br /> facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination <br /> facility on site for three years and have them readily available for review. If the generator did not receive <br /> a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 60 days of the date the waste was accepted by the initial <br /> transporter, the generator shall submit a legible copy of the missing manifest, with some indication that <br /> the generator has not received confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to <br /> DTSC. By November 8, 2010, submit a copy of the signed off copy of the manifests or a copy of the <br /> information submitted to DTSC. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by November 8, 2010. <br /> Note: (1) 55-gallon container of waste gasoline with an accumulation start date of 4/7/10 will have been <br /> stored on site greater than 180 days tomorrow, (1) 25-gallon container of waste brake fluid with an <br /> accumulation start date of 4/11/10 will also be stored on site greater than 180 day in the near future. <br /> These containers should be scheduled to be disposed of by a licensed hazardous waste transporter as <br /> soon as possible. <br /> An Aboveground Petroleum Storage Act survey was conducted and found that this facility has a 1,000 <br /> gallon gasoline tank, a 500 gallon red-dyed diesel tank, and a 385 gallon used oil tank. This facility has <br /> a shell capacity that exceeds 1,320 gallons and therefore a Spill Prevention Control and <br /> Countermeasures Plan must be drafted and implemented and/or the tanks must be permanently closed <br /> per the Code of Federal Regulations. <br /> This facility is going through a business owner change of ownership.
