If using a handheld radio, contact the Fire Department <br />Dispatch through the Central Alarm Station (CAS) by using the call sign "MIKE" on the <br />radio. <br />Note: Cell phones do not function at Site 300. <br />An incident is considered large if any one or more of the following conditions occur: <br />• A fire or explosion occurs, or <br />• Personnel regard the incident as unsafe to manage without the aid of the Fire <br />Department, or <br />• Hazardous, radioactive, or mixed waste with hazards unfamiliar to personnel is <br />released, or <br />• Hazardous, radioactive, or mixed waste that cannot be identified is released, or <br />• There are injuries that require medical treatment other than simple first aid as <br />directed by Health Services, or <br />• The incident requires evacuation of a building or facility, or <br />• Hazardous, radioactive, or mixed waste migrates into a storm drain or sanitary <br />sewer system. <br />1.3 Maintenance of the Plan <br />1.3.1 Copies of the Contingency Plan <br />A copy of the General Plan and the appropriate Site-specific Plan will be maintained <br />onsite at the WAA and in the WAA Coordinator's files. <br />A copy of the General Plan with the Site-specific Plan shall be maintained onsite at: <br />• Fire Department (Site 300 and Livermore site) <br />• EFA Office - Livermore Site <br />• Site 300 Radioactive and Hazardous Waste Management (RHWM) Office <br />• Emergency Operations Center - Livermore Site <br />• Security Organization Assurance Office - Livermore Site <br />• Site 300 Manager's Office REC Vrzll 'ED <br />• Site 300 ES&H Team Environmental Analyst's Office JAN 14 2016 <br />1.3.2 Revisions to the Contingency Plan ENVIRONMENTAL <br />HEALTH DEPA119TMENT <br />This Contingency Plan shall be reviewed periodically by applicable programs/ <br />departments, ES&H Team, and EFA. The ES&H Team Environmental Analyst shall be <br />informed immediately by the responsible program/department if any of the following <br />occur: <br />5 January 2016 <br />
