Box 808, L-627 j <br />Livermore, California 94551 <br />EPA ID Number: CAD 289 009 0002 <br />Dear Mr. Grissom: <br />REPORT OF VIOLATIONS <br />On November 15 and 16, 1993 the Department of Toxic <br />Substances Control (Department) conducted an inspection of <br />Lawrence Livermore National Laboratory - Site 300 (LLNL300). As <br />a result of that inspection a Field Report of Violations (FROV) <br />was issued on November 16, 1993. An additional violation of the <br />California Code of Regulations, Title 22 has been included in <br />this report for that inspection. <br />VIOLATIONS• <br />1. LLNL300 violated Title 22, California Code of <br />Regulations (Cal. Code Regs.) section 66262.34(a)(3) <br />and 66265.32(c), in that on or about November 16, 1993, <br />LLNL300 failed to have decontamination equipment, to <br />wit: <br />The 90 day hazardous waste accumulation area at <br />Building 805 lacked an eyewash/shower. <br />2. LLNL300 violated Title 22, Cal. Code Regs. section <br />66262.34(f)(3), in that on or about November 15 and 16, <br />1993, LLNL300 failed to properly label containers and <br />portable tanks of hazardous waste with the composition, <br />to wit: <br />a. At Building 819, a portable tank used to hold <br />"Wash Down Water" from decontamination operations <br />was not labeled to show the hazardous <br />constituents; <br />b. At Building 801, containers used to hold "Used <br />Photo Chemicals" were not labeled to show <br />hazardous constituents; <br />.41F <br />Panted O Recycied Pppei <br />
