Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />Alameda County, the remainder lies in San Joaquin County. There are no lakes, <br />reservoirs, or rivers within one mile of the EWSF. However, Corral Hollow Creek <br />lies about 300 feet south of the EWSF. <br />11. CONSENT <br />Consent to conduct inspection that involves: taking photographs, reviewing and copying records, <br />questioning personnel and inspecting hazardous waste handling areas. <br />Consent given by: Mr. James Lane, Site 300 Deputy Manager <br />Ill. BACKGROUND <br />PREVIOUS INSPECTION HISTORY: <br />On June 17, July 9, and 13, 1999, a facility Comprehensive Compliance Evaluation (CEI) and <br />hauler inspection resulted in one minor violation. LLNLS300 violated California Code of <br />Regulations, section 66264.16(c) and Facility Permit section VII -C for failure to provide annual <br />refresher for Self -Contained Breathing Apparatus (SCBA)- training. The facility returned to <br />compliance on August 2, 1999. <br />On June 28, 29 and July 12, 2000, a CEI and hauler inspection resulted in one minor violation. <br />LLNLS300 violated California Code of Regulations, section 66264.16(a)(1)(b) and Facility Permit, <br />section VII -C, appendix VII -A, in that on or about June 28, 2000, LLNLS300 had failed to provide all <br />required training to new personnel within six months of the date hired. The facility returned to <br />compliance on August 7, 2000. <br />During the 2000/2001 fiscal year, on May 16, 17, & 18, 2001, a CEI and hauler inspection resulted <br />in one minor violation for violating California Code of Regulations, section 66264.173(x) in that on <br />or before May 17, 2001, the facility failed to keep containers of HtiV closed in the EWSF. This <br />
